Who We Are
Community Health Outreach is a non profit 501 (c) (3) located in the 32210 zipcode of West Jax, which is considered a food and medical desert. We are a Christian based outreach and our mission is to Feed the Hungry, Clothe the Needy, and Heal the Sick.
What We Do
We provide free services for the uninsured and those with low to no income. We have a medical and dental clinic that provides primary care for adults and emergency dental services. Our Food Pantry currently provides groceries for more than 600 families each week. Baby Luv provides free pregnancy testing as well as baby clothes, diapers, wipes, formula and baby equipment.
