Make a meaningful difference in the lives of uninsured Jacksonville residents.

JaxCareConnect is an initiative of the Duval Safety Net Collaborative with a shared vision that every resident of Duval County has access to high-quality, comprehensive healthcare, regardless of insurance status or ability to pay. We help uninsured adults connect with a primary care medical home and identify other barriers that may affect their health and well-being.

As a Patient Experience Volunteer, you will play an important role in helping us understand how our program is working for the people we serve. Through friendly follow-up phone calls, you will listen to patients’ experiences, administer brief one-year surveys, and document feedback that helps our team improve services and strengthen access to care across Jacksonville. If a patient would like additional support or help reconnecting to care, you will notify our team so we can follow up.

Conduct friendly follow-up calls with JaxCareConnect patients.

Administer brief one-year patient experience surveys.

Enter survey responses and patient feedback into our secure system.

Alert staff when a patient requests additional assistance or follow-up.

Minimum commitment: 3–4 hours per week.

Training and orientation are provided before you begin.

Overview:

Help expand access to healthcare by supporting JaxCareConnect’s mission online! As a Digital Impact Ambassador, you'll use your social media presence to share, like, and engage with our content — and help increase community awareness about no-cost and low-cost primary care services for uninsured Duval County residents. Volunteers may also attend local events to capture photo and video content for our social media platforms.

Responsibilities:

- Share and engage with JaxCareConnect content on platforms like Instagram, Facebook, LinkedIn, and TikTok

- Help expand our reach through hashtags, reposts, stories, etc.

- (Optional) Attend community events to capture social media content (photo, video, etc.)

- Stay informed on our services and share our mission with your network

Qualifications:

- At least 18 years

- Passion for healthcare equity and community engagement

- Basic familiarity with social media platforms

- Must reside in or be connected to the Jacksonville area

Overview:

Join JaxCareConnect at health fairs, community resource fairs, and other local events as a Community Connector! Help us engage with the public, distribute materials, and share our mission. You’ll help uninsured residents learn how to access primary care — all while networking with other service organizations.

Responsibilities:

- Represent JaxCareConnect at local community events

- Share information and materials with residents

- Answer basic questions about our services (training provided)

- Help set up/tear down event tables as needed

Qualifications:

- At least 18 years old

- Friendly and outgoing demeanor

- Willing to work occasional weekends

- Interest in health equity and community organizing
