JaxCareConnect's Mission: To connect our uninsured Duval County neighbors to the health care that they need. JaxCareConnect's Vision: Every resident of Duval County has access to high-quality, comprehensive health care – regardless of insurance status or ability to pay.
What We Do
We connect uninsured, Duval County residents with free and charitable health care - through our network of free clinic partners. In addition to that, we evaluate the whole person and explore other barriers to health care, and connect our clients to resources that address those barriers, to support best possible health outcomes.
