JaxCareConnect – Patient Experience Volunteer

Make a meaningful difference in the lives of uninsured Jacksonville residents.

JaxCareConnect is an initiative of the Duval Safety Net Collaborative with a shared vision that every resident of Duval County has access to high-quality, comprehensive healthcare, regardless of insurance status or ability to pay. We help uninsured adults connect with a primary care medical home and identify other barriers that may affect their health and well-being.

As a Patient Experience Volunteer, you will play an important role in helping us understand how our program is working for the people we serve. Through friendly follow-up phone calls, you will listen to patients’ experiences, administer brief one-year surveys, and document feedback that helps our team improve services and strengthen access to care across Jacksonville. If a patient would like additional support or help reconnecting to care, you will notify our team so we can follow up.

What You'll Do

  • Conduct friendly follow-up calls with JaxCareConnect patients.

  • Administer brief one-year patient experience surveys.

  • Enter survey responses and patient feedback into our secure system.

  • Alert staff when a patient requests additional assistance or follow-up.

Time Commitment
